Oral breast cancer drug assistance in 2026: Ibrance, Kisqali, Verzenio and newer pills
The three independent breast cancer copay funds we checked were closed on 26 October 2026, so the open doors are the makers: copay offers for commercial insurance, free-drug programmes with published income limits, and one 5-month bridge.
On 26 October 2026 all three independent breast cancer copay funds we checked (CancerCare breast, CancerCare metastatic breast, HealthWell Breast Cancer Medicare Access) were closed to new patients, so for oral breast cancer pills the open door is the maker: free-drug ceilings are $47,880 for one person at Pfizer (300% of the poverty level, for Ibrance) and $79,800 at Lilly Cares (500%, for Verzenio and Inluriyo), and the Ibrance and Kisqali copay offers both exclude Medicare.
Method: we read each maker’s own programme pages, the Lilly Cares application and the three fund pages on 26 October 2026, converted dollar limits to the 2026 HHS poverty guideline ($15,960 for one person in the 48 states), and recorded what each prints and leaves out. How we built thisQuick answer: what help exists for oral breast cancer medicines?
By insurance type:
- Commercial insurance: Ibrance savings of $4,000 to $10,000 a year, Kisqali Co-Pay Plus, and Truqap’s co-pay savings (no income requirements) each say as little as $0.
- Insurer delays or denies Kisqali: its Bridge Program provides up to 5 months free, for private insurance only.
- Medicare or no insurance: the copay offers do not apply. Free-drug routes: Pfizer Patient Assistance Program (Ibrance, up to 300% of the poverty level, including Medicare Part D patients), Lilly Cares Group 4 (Verzenio and Inluriyo, up to 500%), and the Novartis foundation (Kisqali).
- Independent funds: CancerCare’s breast and metastatic breast funds and HealthWell’s breast cancer fund were all closed on 26 October 2026.

How do you get them free without insurance?
Pfizer’s Ibrance page says that if Extra Help or other funding is not available, Pfizer Oncology Together will see if you qualify for the Pfizer Patient Assistance Program, which provides prescribed Pfizer Oncology medicines free. Pfizer’s programme update says that for 2026 all patients, including Medicare Part D and Medicare Advantage patients, must have a pre-tax household income at or below 300% of the poverty level, which is $47,880 for one person in the 48 states. Uninsured patients are first checked for Medicaid. Our Pfizer guide has the application steps.
Lilly Cares’s 2026 application puts Verzenio and Inluriyo in Group 4: income at or below 500% of the poverty level ($79,800 for one person, $108,200 for two, $136,600 for three, $165,000 for four). Group 4 patients may be uninsured, on Medicare Part D, on Medicare Part B with no supplemental cover, or insured where the plan does not cover the medicine; applicants cannot be enrolled in Medicaid, full Extra Help or VA benefits. Our Lilly Cares guide covers the programme in full.
Novartis’s foundation lists Kisqali (1-800-277-2254); its pages print no dollar income limit, and our income limits comparison records 400% ($63,840 for one person) from the programme dataset checked 19 October 2026.
Foundation help: CancerCare and HealthWell
Three independent breast cancer copay funds were closed on 26 October 2026. CancerCare’s breast cancer fund and its metastatic breast cancer fund each read “Status: Closed,” with an email sign-up for re-opening; they require insurance and an income at or below 500% of the poverty level, and CancerCare’s FAQ says assistance from its funds can count toward true out-of-pocket costs for the Medicare Part D annual cap. HealthWell’s Breast Cancer Medicare Access fund also read “Closed,” with a $7,500 maximum award and the same 500% income limit; abemaciclib is on its covered-treatment list. We link the live fund pages rather than assert a fund is open.
What if you have Medicare? Ibrance’s 2027 price and the $2,400 cap
The Ibrance and Kisqali copay offers exclude Medicare, so Medicare patients rely on the free-drug routes above, Extra Help and Part D’s own limits. Medicare has negotiated a price for Ibrance that takes effect in 2027: CMS’s fact sheet lists a 30-day negotiated price of $7,871 against a list price of $15,741 (50% lower). That is the price Medicare plans pay, not your copay: out-of-pocket spending on covered Part D drugs is capped at $2,400 in 2027 (Medicare.gov). Our 2027 negotiated prices guide covers all 15 drugs. All seven medicines above were listed on all 328 Part D formularies in CMS’s September 2026 file; a listing still does not tell you whether prior authorization applies.
What should you ask when you call?
Medicine: ______________ Insurance type: ______________ Date: ________
Commercial insurance:
[ ] Is the copay offer open to me? What is the yearly limit? What ends it?
[ ] Insurer delaying or denying? Is there a bridge? (Kisqali: up to 5 months; file within 90 days)
[ ] Does a copay maximizer or accumulator plan change what the offer pays?
Medicare, Medicaid, or no insurance:
[ ] Which free-drug programme covers this medicine and what is the income limit?
(Pfizer: 300% of the poverty level, $47,880 for one person. Lilly Cares Group 4: 500%, $79,800.)
[ ] Medicare: do I need an Extra Help denial first?
Independent funds: check CancerCare and HealthWell for a re-opened fund, and sign up for alerts.

- If you have commercial insuranceEnrol in the maker’s copay offer on the day your oncologist chooses the medicine, and ask the hub whether a copay maximizer or accumulator plan changes what it pays.
- If you have MedicareNo copay offer applies. Ask the free-drug programme for your medicine (Pfizer takes Medicare Part D patients up to 300%; Lilly Cares takes them up to 500%), and check Extra Help, which Pfizer and Novartis both ask about first.
- If you have no insuranceApply to the maker’s free-drug programme and check Medicaid at the same time; Pfizer checks Medicaid first.
- If you were counting on a foundation grantAll three independent breast cancer funds are closed today; register for the re-opening alerts and use a maker route meanwhile.
We read each programme’s own eligibility page, application form or terms on 26 October 2026 (Ibrance and Pfizer’s programme update, Kisqali, Itovebi, Truqap, Orserdu, the Lilly Cares 2026 application, CancerCare and HealthWell fund pages), converted dollar limits to percentages of the 2026 HHS poverty guideline, and drew on the CMS 2027 negotiated price fact sheet (compared byte for byte with our saved copy) and CMS’s September 2026 Part D formulary file. We did not apply to any programme. Where a programme does not publish a figure, we say so rather than estimate it; Lilly’s oncology sites were unreachable and are not quoted. We compare support programmes, never treatments. OmniRx does not employ named credentialed reviewers; this is patient-navigation reference, not medical advice. Full policy.

Frequently asked questions
Are there free programmes for oral breast cancer drugs without insurance?
Yes, from the makers. Pfizer's Patient Assistance Program (Ibrance) requires income at or below 300% of the 2026 poverty level, $47,880 for one person; Lilly Cares (Verzenio and Inluriyo, Group 4) allows up to 500%, $79,800 for one person; the Novartis Patient Assistance Foundation lists Kisqali. Each decides eligibility itself.
Can I use the Ibrance or Kisqali copay card with Medicare?
No. Ibrance's savings are not for patients enrolled in state or federal programmes including Medicare, Medicaid, TRICARE and VA, and Kisqali's Co-Pay Plus is not valid under Medicare, Medicaid, TRICARE or VA. Medicare patients use the free-drug routes and Extra Help.
Is the CancerCare or HealthWell breast cancer fund open?
Not on 26 October 2026. CancerCare's breast cancer and metastatic breast cancer funds and HealthWell's Breast Cancer Medicare Access fund each read "Closed" on their own pages. Each has a 500% poverty-level income limit. Check the live pages, which change as donations arrive.
Does Kisqali offer a bridge if my insurer delays coverage?
Kisqali's Bridge Program provides up to 5 months free for patients with private insurance and a denial based on a prior authorization requirement, if a prior authorization or appeal is submitted within the first 90 days. It is not available to patients on Medicare, Medicaid or TRICARE.
What is Ibrance's Medicare price in 2027?
CMS's fact sheet lists a 30-day negotiated price of $7,871 for Ibrance in 2027 against a list price of $15,741, which is 50% lower. That is what Medicare plans pay; a Part D enrollee's yearly out-of-pocket spending on covered drugs is capped at $2,400 in 2027.
The bottom line
For oral breast cancer medicines the open doors are the makers: commercial copay offers (Ibrance $4,000 to $10,000 a year; Kisqali and Truqap as little as $0), a 5-month Kisqali bridge, and free-drug programmes with a 300% ceiling at Pfizer and 500% at Lilly Cares. The three independent breast cancer funds we checked were closed on 26 October 2026, and none of the copay offers works with Medicare.
