Eligibility Requirements

Income Limit
There is no single Lilly Cares threshold, it depends on the medication group. Lilly sorts products into four groups: Group 1 at or below 300% FPL, Group 2 at 400%, Groups 3 and 4 at 500% (household annual adjusted gross income). The flat percentage previously shown here is only correct for one group. ⭐ Medicare Part D enrollees are eligible, not only the uninsured, and for Groups 3 and 4 so are Medicare Part B patients without supplemental cover. Part D enrolment expires at the end of the calendar year and must be renewed. Verified 23 August 2026.
Insurance Requirements

Sources and standards behind this page

Drug coverage in the United States is governed by published rules rather than by any one insurer's summary, and the sources below are the primary ones. They are linked so a reader can check a claim without taking this page's word for it.

This page describes how coverage and formularies work. It is not medical advice, and a reader's own plan documents and prescriber decide what applies to them.

Frequently Asked Questions

Can I get free Mounjaro?

Yes. Lilly Cares sets income limits by medication group rather than one figure (300%, 400% or 500% of the Federal Poverty Level depending on the group), and Medicare Part D enrollees are eligible as well as uninsured patients. Check which group Mounjaro falls into before assuming you are over the limit. Your healthcare provider must submit the application with proof of income.

Is there a Mounjaro copay card?

Yes, commercially insured patients with a Mounjaro prescription for type 2 diabetes may qualify for the Mounjaro savings card, which can reduce copays to as low as $25 per month.

Does insurance cover Mounjaro?

Many commercial insurance plans cover Mounjaro for type 2 diabetes. Coverage for weight management may be limited. Check with your insurer, and use the savings card or PAP if coverage is denied.
