How This Interaction Works (Mechanism)
Prednisone suppresses prostaglandin synthesis and reduces the mucus and bicarbonate protective layer of the stomach. It also impairs tissue healing in the GI tract. Ibuprofen further inhibits COX-1-mediated prostaglandin production in the stomach lining. Together, they leave the gastric mucosa with virtually no prostaglandin protection, making it highly vulnerable to acid damage, erosion, and bleeding.

Avoid taking ibuprofen or any NSAID while on prednisone unless specifically directed by your doctor. Use acetaminophen for pain relief instead. If the combination is medically necessary, your doctor may prescribe a proton pump inhibitor (such as omeprazole) to protect your stomach. Report any stomach pain, dark stools, or vomiting blood immediately.
