How This Interaction Works (Mechanism)
Lexapro (escitalopram) blocks the serotonin transporter (SERT), preventing serotonin reuptake. Tramadol also inhibits serotonin reuptake and may increase serotonin release. Together, they cause excessive serotonin accumulation in the synaptic cleft, overstimulating 5-HT receptors (particularly 5-HT2A). This can trigger serotonin syndrome, characterized by autonomic instability, neuromuscular hyperactivity, and altered mental status.

Avoid this combination when possible. If both medications are necessary, your doctor should start with the lowest effective doses and monitor closely for signs of serotonin syndrome: agitation, confusion, rapid heart rate, high blood pressure, dilated pupils, muscle twitching, diarrhea, and high fever. Seek emergency medical care if these symptoms develop. Alternative pain medications that do not affect serotonin should be considered.
