What Happens When You Take Amoxicillin and Birth Control Together

The concern that amoxicillin reduces the effectiveness of hormonal birth control is widespread but not well-supported by clinical evidence. Most studies show no significant reduction in contraceptive hormone levels when taking amoxicillin. However, GI side effects from the antibiotic (vomiting, diarrhea) could theoretically reduce absorption of oral contraceptives.

How This Interaction Works (Mechanism)

The theoretical mechanism involves disruption of the enterohepatic recirculation of ethinyl estradiol. Some gut bacteria produce enzymes that deconjugate estrogen metabolites excreted in bile, allowing them to be reabsorbed. Antibiotics that kill these bacteria could reduce this recycling. However, clinical studies of amoxicillin have not shown meaningful reductions in blood levels of contraceptive hormones.

What You Should Do

Current medical guidelines (ACOG, WHO) state that non-rifamycin antibiotics like amoxicillin do not reduce the effectiveness of hormonal contraception. However, if amoxicillin causes vomiting or severe diarrhea, absorption of oral contraceptives may be affected. In that case, use backup contraception (condoms) for the remainder of your pill pack.
