How This Interaction Works (Mechanism)
Ibuprofen inhibits COX-1, which reduces platelet aggregation and disrupts the protective prostaglandin lining of the GI tract. It can also displace warfarin from plasma protein binding sites, temporarily increasing free warfarin levels. Additionally, ibuprofen may inhibit CYP2C9, one of the enzymes responsible for warfarin metabolism, further increasing warfarin's anticoagulant effect.

Avoid ibuprofen and all NSAIDs while taking warfarin. Use acetaminophen for pain relief instead (but limit to less than 2 grams per day, as high-dose acetaminophen can also affect INR). If NSAID use is unavoidable, use the lowest dose for the shortest time and add a proton pump inhibitor for GI protection. Notify your doctor and get more frequent INR monitoring.
