What Happens When You Take Warfarin and Acetaminophen Together

Regular use of acetaminophen (more than 2 grams daily for several days) can increase the anticoagulant effect of warfarin and raise the INR, increasing bleeding risk. While acetaminophen is generally recommended over NSAIDs for warfarin patients, high doses still require caution.

How This Interaction Works (Mechanism)

Acetaminophen's active metabolite, N-acetyl-p-benzoquinone imine (NAPQI), inhibits vitamin K-dependent carboxylase enzymes, which are needed to activate clotting factors. At higher doses, enough NAPQI accumulates to reduce the activity of clotting factors II, VII, IX, and X, potentiating warfarin's anticoagulant effect. The interaction typically becomes clinically significant at acetaminophen doses above 2 grams per day for more than 3 consecutive days.

What You Should Do

Acetaminophen remains the preferred pain reliever for patients on warfarin, but limit use to less than 2 grams (2,000 mg) per day. If regular acetaminophen use is needed, check your INR after 3 to 5 days. Occasional single doses of acetaminophen are unlikely to cause problems. Avoid combination products that contain both acetaminophen and other active ingredients, as they may add to the risk.
