What Happens When You Take Spironolactone and Potassium Together

Spironolactone is a potassium-sparing diuretic that blocks aldosterone receptors, directly preventing the kidneys from excreting potassium. Taking potassium supplements on top of this effect can cause life-threatening hyperkalemia (dangerously high potassium levels) leading to cardiac arrest.

How This Interaction Works (Mechanism)

Spironolactone blocks mineralocorticoid (aldosterone) receptors in the renal collecting duct. Aldosterone normally stimulates sodium reabsorption and potassium excretion through epithelial sodium channels (ENaC). When aldosterone is blocked, the kidneys retain potassium instead of excreting it. Adding exogenous potassium to this already-impaired excretion creates a high risk of hyperkalemia (serum K+ above 5.5 mEq/L), which can cause fatal ventricular fibrillation.

What You Should Do

Do not take potassium supplements or use potassium-containing salt substitutes while on spironolactone unless explicitly directed by your doctor with regular monitoring. Have your potassium and kidney function tested within 1 week of starting spironolactone and regularly thereafter. Limit high-potassium foods like bananas, oranges, and potatoes in excessive quantities. Seek immediate medical attention for muscle weakness or palpitations.
