What Happens When You Take Prednisone and Ibuprofen Together

Taking prednisone with ibuprofen significantly increases the risk of gastrointestinal bleeding and peptic ulcers. Both drugs independently damage the stomach lining, and their combined effect creates a much higher risk of serious GI complications, including perforation and hemorrhage.

How This Interaction Works (Mechanism)

Prednisone suppresses prostaglandin synthesis and reduces the mucus and bicarbonate protective layer of the stomach. It also impairs tissue healing in the GI tract. Ibuprofen further inhibits COX-1-mediated prostaglandin production in the stomach lining. Together, they leave the gastric mucosa with virtually no prostaglandin protection, making it highly vulnerable to acid damage, erosion, and bleeding.

What You Should Do

Avoid taking ibuprofen or any NSAID while on prednisone unless specifically directed by your doctor. Use acetaminophen for pain relief instead. If the combination is medically necessary, your doctor may prescribe a proton pump inhibitor (such as omeprazole) to protect your stomach. Report any stomach pain, dark stools, or vomiting blood immediately.
