What Happens When You Take Levothyroxine and Coffee Together

Coffee can reduce the absorption of levothyroxine when taken at the same time. Drinking coffee immediately after taking levothyroxine can decrease the drug's absorption by up to 30 percent, potentially leading to inadequate thyroid hormone levels and the need for higher doses.

How This Interaction Works (Mechanism)

Coffee stimulates gastric motility and increases intestinal peristalsis, which reduces the transit time of levothyroxine through the absorptive regions of the small intestine. Coffee also increases gastric acid secretion, which may alter the dissolution of levothyroxine tablets. Some studies suggest that coffee's polyphenols may bind to levothyroxine, reducing its bioavailability. The effect is most pronounced with espresso and unfiltered coffee.

What You Should Do

Wait at least 30 to 60 minutes after taking levothyroxine before drinking coffee. Take levothyroxine with a full glass of plain water on an empty stomach immediately upon waking. If you find it difficult to wait before your morning coffee, ask your doctor about taking levothyroxine at bedtime instead (at least 3 hours after your last meal). Liquid or gel cap formulations of levothyroxine may be less affected by coffee.
