What Happens When You Take Fluoxetine and Tramadol Together

Combining fluoxetine (Prozac) with tramadol creates a high risk of serotonin syndrome and also reduces tramadol's pain-relieving effectiveness. This is a dual problem: the combination is both more dangerous and less effective for pain management.

How This Interaction Works (Mechanism)

Both fluoxetine and tramadol inhibit serotonin reuptake, creating risk of serotonin toxicity. Additionally, fluoxetine is a potent CYP2D6 inhibitor, and tramadol requires CYP2D6 to convert it to its active metabolite (O-desmethyltramadol, or M1), which is responsible for most of its analgesic effect. Fluoxetine blocks this conversion, reducing tramadol's pain relief by 50 to 80 percent while simultaneously increasing serotonin syndrome risk.

What You Should Do

Avoid this combination. If pain management is needed while taking fluoxetine, use alternatives that do not affect serotonin, such as acetaminophen, NSAIDs (with GI precautions), or non-serotonergic opioids if severe pain requires them. If this combination has been prescribed, watch for serotonin syndrome symptoms: agitation, confusion, tremor, rapid heart rate, fever, and muscle rigidity. Seek emergency care if these develop.
